Thank you for visiting the Medical Records Department of Haskell County Hospital. We are happy to help you with any medical record needs.
Our department is responsible for all emergency room and inpatient hospital service records. Every time a patient receives healthcare, a record is maintained of the observations, medical or surgical interventions and treatment outcomes. This record includes information that the patient provides concerning his or her symptoms and medical history, the results of examinations, reports of x-rays and laboratory tests, diagnoses and treatment plans. Medical record employees organize and evaluate these records for completeness and accuracy. The Medical Records department is also responsible for the storage and retrieval of medical records and providing statistical information for the administrative staff.
Your medical records are covered by HIPAA privacy regulations and our own hospital confidentiality standards as stated in the Haskell County Healthcare System Notice of Privacy Practices.

We understand that medical information about you and your health is personal. We are committed to protecting medical information about you. We create a record of the care and services you receive at Haskell County Healthcare System. We need this record to provide you with quality care and to comply with certain legal requirements. This notice applies to all of the records of your care generated by Haskell County Healthcare System, whether made by HCHS personnel or your attending physician. You personal doctor may have different policies or notices regarding the doctor's use and disclosure of your medical information created in the doctor's office or clinic.We are required by law to
